• Coordinate care with other professionals (with your consent)
• Maintain accurate records as required by law and professional standards
• Ensure safety planning and crisis support when needed

How Your Information Is Stored
All information is stored securely in encrypted, password-protected digital files or locked physical files. Only the social worker providing services has access to your records. Records are maintained in accordance with ACSW standards and Alberta legislation.
Confidentiality and Its Limits
Your information is confidential except in the following situations required by law:
• Risk of harm to yourself or someone else
• Suspected child abuse or neglect, as required under the Child, Youth and Family Enhancement Act
• Disclosure required by court order
• Situations where disclosure is necessary to protect someone’s safety
Whenever possible, you will be informed before any required disclosure is made.
Consent and Your Rights
You have the right to:
• Ask questions about how your information is collected, used, or shared
• Withdraw or change your consent (except when disclosure is required by law)
• Access your records
• Request corrections to your information
• Ask how long information is kept and how it is destroyed
Consent for collection and sharing of information is obtained verbally or in writing.
Third-Party Services
If technology (such as email, video sessions, or online forms) is used, all reasonable steps are taken to ensure privacy.
